By signing this waiver, I affirm that I am the utility account owner listed on the utility account of and/or reside at the property listed, and therefore a recipient of the services provided by the City of Tampa's (City) recycling program.

I understand and agree to comply with all applicable City requirements, including those outlined in the City Code and Program Rules and Requirements.

I acknowledge that completion of the City-sponsored Recycling Workshop is required in order to reinstate recycling service and receive a recycling cart. I affirm that I have successfully completed the Recycling Workshop.

I understand that the recycling cart issued to me remains the property of the City of Tampa.

I accept full responsibility for ensuring the proper use of the cart and for the contents placed within it.

Misuse of the recycling cart, including, but not limited to, placing garbage or other non- recyclable materials in the cart, may result in the removal of the cart and revocation of recycling service. I acknowledge that if the recycling cart is removed and recycling service is revoked due to misuse, the City is not obligated to provide a recycling cart or restore recycling service.

Furthermore, I understand that the City is not liable for any claims, damages, losses, injuries, liabilities, or demands that may arise from or be connected to my control, use, or misuse of the recycling cart.
